Each person or family member must register separately for their own IV Quad session. Children under 13 must have an adult guardian with them at all times. Registering signifies agreement to the Makerspace Liability Waiver for you and/or any minor children:

It’s the season of love! This month’s project is all about creating heart-shaped puzzles using sublimation. You can either use a personal photo or design your own unique image. To use a personal photo, make sure it’s on a thumb drive or uploaded to our Quad computer. Limited one project per person. 

I, hereby for myself (and/or any minor children), on behalf of my heirs, successors, and assigns, in consideration of being permitted to use tools and equipment, agree to release and hold harmless the Washoe County Library System and Washoe County Government, its officers, employees, volunteers, committees and boards, from and against any and all liability, loss, damages, claims, or actions (including costs and attorney fees) for bodily injury, illness, death and/or property damage, to the extent permissible by law. 

This release, indemnification and hold harmless agreement shall include indemnity against all costs (including without limitation, reasonable attorney fees and court costs), expenses and liabilities incurred in, or in connection with any such claim or proceeding brought thereon and in defense thereof.

I have read and understand this release, indemnification and hold harmless form. I voluntarily accept it for myself (and/or my minor children), and hereby give permission to the Washoe County Library System and Washoe County Government to facilitate emergency transportation and/or treatment in the event of illness or injury.

I hereby accept responsibility for the payment of any emergency transportation and/or treatment.   I further certify that I (and/or my minor children) am/are in good physical condition and have no medical or physical conditions that would restrict participation in activities or programs in The IV Quad or other Washoe County Library System makerspace locations.

I certify that I have read this waiver and I fully understand its content. I am aware that this is a release of liability and a contract, and I accept it of my own free will.
